A free Mini Blizzard is not a guaranteed reward for every Dairy Queen survey. Check the offer printed on your own receipt or coupon. If it specifically mentions a Mini Blizzard, use the survey website and follow the redemption instructions printed there. If it mentions a different item, the Mini Blizzard offer does not replace that item.

How to check your Mini Blizzard offer

  1. Find the survey invitation and offer on your full receipt. Read the item, size, participating-location wording, expiration date and any purchase requirement that are actually printed.
  2. Open the survey website shown on the receipt. The official entry form may ask for a 10-digit store phone number plus visit date and time, a 15-character code, or a 19-digit code, depending on the website.
  3. Complete the survey if your receipt is eligible. Follow any instructions on the final screen. If it provides a validation code and asks you to write it on the receipt, keep the original receipt and do so.
  4. Take the receipt or coupon and any required validation information to an eligible DQ location before the date printed in your offer. The receipt terms and the store determine whether it can be redeemed.

Choose the survey page printed on your receipt

Do not enter a 19-digit code into the 10-digit phone form. A survey code printed on a receipt is the information used to start a survey; a validation code, if supplied after completion, is a separate item. Our DQ survey rewards guide explains the formats in more detail.

If the survey code does not work

Check the exact website on your receipt, all digits and letters, and the receipt’s date and time. The 15-character entry page says not to use the letter O. Dairy Queen says that when a QR code or 15-character code produces an error, you can try the alternate entry using the store’s 10-digit phone number from the receipt. If the problem continues, use the contact form linked in the official Dairy Queen FAQ and include a photo of the receipt.

What if there is no Mini Blizzard on my receipt

Completing a survey does not by itself create a Mini Blizzard coupon. Check the offer that came with your receipt and any terms displayed after the survey. Current app deals and DQ Rewards are separate programs; use Dairy Queen’s official rewards page to check offers currently available to your account and location.
