MyDQExperience Survey Code and Receipt Guide

If your Dairy Queen receipt says MyDQExperience.com, use the 15-character survey code printed on that receipt. Enter it on the official MyDQExperience.com form. The site says not to use the letter O in a survey code. Keep the receipt until you have checked any offer or follow-up instructions.

Where to find the code

Look near the survey invitation on the front or back of your DQ receipt. Find a 15-character group labeled “survey code.” Do not use an order number, store number or a possible validation code from an earlier survey. How to take the MyDQExperience survey

  1. Read the receipt’s survey invitation and check its printed eligibility and deadline. Open MyDQExperience.com in your browser.
  2. Type the complete 15-character code as printed. Check zero and other similar-looking characters; the official page specifically says not to use the letter O.
  3. Continue to the questions and provide your own feedback about the visit.
  4. Read the final screen and the offer on your receipt. If you receive a validation code and are instructed to write it down, copy it on the original receipt and keep it for redemption according to the printed terms.

If the code or QR link gives an error

First check that the page really is MyDQExperience.com and that you entered the right code. The official DQ FAQ advises trying the link for entry by the restaurant’s 10-digit phone number when a QR code or 15-character survey code errors; the phone number is usually near the top of the receipt. Its FAQ also asks you to use the DQ contact form with a photo of the receipt if the problem continues. The main DQFanFeedback.com form asks for that 10-digit phone number plus the visit date and time.

What can you get for completing it

Check the offer on your own receipt. A survey invitation does not guarantee a Dilly Bar, Mini Blizzard or any other specific reward. Choose the site named on your receipt. A 19-digit code will not fit the MyDQExperience form. Do not put receipt codes into an unrelated website.

Can I submit without a receipt? The official entry form requires the receipt’s 15-character survey code. Keep the original receipt for any offer terms.

Is the survey code a reward code? No. It starts the survey. A separate validation code, if any, may appear after you finish.
