The reward for a Dairy Queen receipt survey depends on the offer printed on that receipt. A free Dilly Bar or Mini Blizzard is possible only when the applicable receipt or coupon actually offers it. The survey page itself is not proof that every guest gets the same item. Read the offer, expiration and redemption terms before you start.

Find the right official survey
- If the receipt directs you to DQFanFeedback.com, the main form asks for the 10-digit store phone number on the receipt and your visit date and time. If your receipt has no 10-digit number, the site offers an 15-character alternate entry.
- If the receipt directs you to MyDQExperience.com, enter the 15-character survey code printed on that receipt. The entry form says not to use the letter O.
- If the receipt directs you to DQFanSurvey.com, enter its 19-digit survey code. This is a separate form.

How a receipt offer works
- Locate the offer and survey invitation. Check the item, any required purchase, the participating store or region and all printed dates.
- Open the matching official website above, enter the requested receipt information and submit your honest feedback.
- Follow the final survey screen. If a validation code is displayed and the receipt tells you to write it down, copy it carefully onto the original receipt.
- Bring the original receipt or coupon and any required code to the eligible store within the time and conditions shown on your offer.
Survey code and validation code
The survey code is printed on a qualifying receipt and opens the questionnaire. A validation code, if provided, appears at the end of a completed survey and may be required to claim the printed offer. They are not interchangeable. Do not share either code publicly.
Which reward will I receive

If your receipt or code is rejected
Recheck the website on the receipt, the visit date and time, and each character. On a 15-character entry form, do not substitute the letter O for a zero. The official Dairy Queen FAQ says that if a QR code or 15-character survey code errors, try entering the restaurant’s 10-digit phone number instead. If you still cannot complete the survey, contact DQ using the form linked there and attach a receipt photo. Your receipt or coupon controls any expiration; there is no single time limit for every offer.
